This form is for individuals wishing to register for the PMC Student Service. In order to receive the services outlined, you must register with PMC and pay an up-front fee. The fee is currently £500 with secure online payment processing provided by PayPal.

Please include as much detail as possible when completing this form. The information you provide will be used as the basis for your Individual Report and subsequent support.

Please select a username which must be a valid email address and a password which should be a minimum of 8 characters and be composed of numbers and letters with a number somewhere in the middle. You should also avoid using the same character repeatedly.
